JMPでBland-Altman分析:測定方法の一致性を評価する手法

Bland-Altman分析は、2つの測定方法の一致性を評価するための統計的手法であり、主に医療や製造の分野で広く用いられています。以下に示すのは、Bland-Altaman分析を行う際に使用される「Bland-Altmanプロット」というグラフです。このプロットでは、縦軸に2つの測定値の差を、横軸に2つの測定値の平均をプロットすることで、測定間のバイアス(偏り)や一致性を評価できます。

JMPでは、バージョン17で「対応のあるペア」のオプションとしてBland-Altman分析が搭載され、最新のバージョン18では機能がさらに追加されています。

 

本ブログでは、Bland-Altman分析の概要とJMPを使った分析の手順、注意点について説明します。

 

使用するデータ

JMPのサンプルデータ「Method Comparison.jmp」を用います。このデータには、20個の対象物について「標準の方法」と「測定方法1」で測定した結果が含まれています。これらのデータを用いて、測定方法間の一致性を評価します。

 

このデータセットでは、各対象物について標準の方法と測定方法1の測定結果が対応しているので、対応のあるデータです。「グラフビルダー」を使って、横軸に標準方法、縦軸に測定方法1をプロットした散布図、横軸と縦軸の値が一致する直線(青色)を描いています。

プロット点が直線上にあれば、双方の測定値が一致していることを意味します。この例では、多くのデータ点は直線付近にプロットされていますが、いくつかの点が直線からずれているプロットがいくつかあることが確認できます。

 

Bland-Altman分析では、2つの測定値の差が正規分布に従うことを前提としています。

 

Bland-Altman分析

上記のデータのように、比較する測定値が2列にわたって入力されているデータについて、JMPではBland-Altman分析を次の手順で実行します。

 

  1. [分析] > [発展的なモデル] > [対応のあるペア ] にて、比較する2つの列を [Y,対応のある応答] に指定します。
  2. レポート「Y, 対応のあるペア」の左にある赤い三角ボタンから [Bland Altman分析] を指定します。

レポート下側に「Bland-Altman分析」のレポートが表示されます。

Bland-Altman分析では、上図の赤枠で示した値が重要です。

 

バイアス:測定方法1と標準方法の差に対する平均。各データにおける「測定方法1の値 - 標準方法の値」を計算し、それらの値の平均を求めたものです。この例では、バイアスは0.113です。

許容限界の下限、上限:測定が一致すると許容できる区間幅です。平均の差が正規分布に従っていると仮定できるとき、約95%のデータがこの範囲に収まることが期待されます。JMPでは標準正規分布の97.5%点 (Z(0.975))を使って、次のように計算されます。

 

バイアス ± Z(0.975) * (バイアスの標準偏差) 

(注:Z(0.975) の値は1.95996ですが、Bland-Altman分析では簡便的に1.96という値を用いるケースが多くみられます。)

 

この例における 許容限界は、(-3.229, 3.455) です。

 

Bland-Altmanプロット

Bland-Altmanプロットは、上記のBland-Altman分析で算出したバイアス、許容限界をプロットし、測定間の一致性を視覚的に確認できるグラフです。

 

注意点として、JMPの「対応のあるペア」のレポートに表示されるグラフはBland-Altmanプロットそのものではありません。このグラフの赤い点線が許容限界だと思われがちですが、これは平均の差に対する95%信頼限界を示しており、標準誤差に基づいて算出されたものです。

 

一方、Bland-Altmanプロットの許容限界は標準偏差をもとに算出されるのため、このプロットを描くには、次の手順(1), (2)でレポートのグラフを編集する必要があります。

 

(1) 信頼区間の点線を非表示に

「対応のあるペア」のレポートに表示されるグラフを右クリックし、メニューから [カスタマイズ] を選択します。

「グラフのカスタマイズ」ウィンドウで次の設定を行います。

  •  左側の項目「上側の線」、「下側の線」を上矢印のアイコン(↑)を何度かクリックして上側に移動します。
  •  線の色をクリックし、色を赤から白色に変更します。

(2) 許容限界線の設定

グラフの縦軸をダブルクリックし、Y軸の設定で許容限界の下限、上限を参照線として設定します。下図は参照線を設定する例です。

これらの手順で、以下のBland-Altmanプロットを作成できます。赤の実線はバイアスの値を示しています。

前述の通りプロット点は、以下の計算で算出されています。

縦軸:2つの測定値の差

横軸:2つの測定値の平均

縦軸は測定値の差なので、差がほとんどないのであれば Y=0 付近にプロットされます。測定の差が許容範囲の下限と上限(青の点線)内に収まれば、一致性が良好であると判断できます。この例では、1つの点を除き、すべての点が許容範囲内に収まっており、概ね一致性は良好です。ただし、範囲外の点については、データに戻り原因を検討する必要があります。

 

横軸の測定方法の平均値に対して、縦軸の差がどのように変化するかを考察することも重要です。下図のように測定値が大きくなるほど差が大きくなる場合、比例誤差が存在することになります。このような場合、単純な平均差で一致性を評価するのは難しくなります。

変化率で一致性を評価(JMP 18の新機能)

ここまで示したBland-Altman分析は、2つの測定値の差を用いて一致性を評価してきましたが、変化率(ある測定法が別の測定法に対して何パーセント変化したか)を用いることもあります。例えば、2種類の体重計(A,B)で測定した際、Bの体重計がAに対して0.1%増加したなどのケースです。

 

変化率で考えることは、上記で示した比例誤差が存在する際の一つの対処方法にもなります。

 

最新版の「JMP 18」では、測定間の一致性を変化率で検討できる機能が追加されています。レポート「対応のあるペア」の赤い三角ボタンから、変化率を分析するオプション(下図の赤枠部分)を選択できます。
