To me that doesn't really seem to be a JMP problem but rather with your SQLPathfinder application. Have you tried what the error message suggest (check configure form)? Or have you contacted SQLPathfinder developers?
Now that you mentioned it, I will reach out to the SQL Pathfinder folks to see if they have a solution or something - I am unable to locate the "Configure Form", so it's hard to try troubleshooting it.
