済生会病院における医師の働き方に関する実態調査データの分析_佐村 紫帆(2023-JA-PO-05)

【発表概要】

本研究は、医師の働き方を適正化するための2024年4月に施行される医師の働き方改革に向けて実施された調査データを分析したものである。本研究の調査対象病院は、日本の社会福祉法人としては最大の公的医療機関を運営する恩賜財団済生会(許可病床数22;191床)の81病院とした。そのうち76病院・1468名の常勤医師から無記名自己記入式質問紙調査で回答を収集した(回答率35%)。公開されている病床機能報告における病床機能区分やアンケート回答医師の分布に留意し、Classification and regression tree (CART)による回帰木を作成した。分析の結果として所属病院、医師の疲労の程度、休日の疲労回復の程度が最も職務満足度に影響を与えており、勤務時間や兼業病院の有無、当直やオンコールの回数など自己申告で把握可能な指標は直接的にはあまり大きな影響を与えていないことがわかった。医師の健康状態が保たれ、職務満足度を向上するとともに、質の高い医療を永続的な形で提供できるように労務管理を検討する必要があると結論づけた。

 

【発表者プロフィール】

私は看護医療学部学2年次からクリニックにおける初診患者満足度調査に取り組み、研究成果を卒業論文にまとめDiscovery Summit Japan 2021で口頭発表しました(2021-JA-25MP-08)。現在大学院では医療マネジメントを専攻し、「医療政策・管理学」や「データ対話型病院経営論」などの授業を通してデータに基づく政策決定・病院経営について学んでいます。大学院1年次には、医療の質評価に関して医療事故発生割合の要因分析を発表しました(2022-JA-25MP-07)。2年次になり修士論文では、2024年に施行される医師の働き方改革に関して、恩賜財団済生会が運営する病院の常勤医師の働き方と健康状態、職務満足度のデータ分析に取り組んでいます。来年度から新卒看護師として、地元沖縄の救命救急センターで従事し、臨床現場で研究的視点を持って活躍したいと意気込んでいます。
